Authorisation for acting on behalf of another party
If you find it difficult to manage your affairs at Aava in person, you can give another person a power of attorney with which you authorise them to manage your affairs at Aava on your behalf. The authorisation must be made in writing by using a form template or a free-form power of attorney. The free-form power of attorney must include:
- the name, identity code and contact information of the person issuing the power of attorney
- the name, identity code and contact information of the authorised person, as well as information on their relationship to the authorising party
- as detailed a definition as possible of the services to which the authorisation applies
- validity of the authorisation
We always verify the identity of the person granting the authorisation at Aava in connection with the delivery of the power of attorney. If the person to whom the power of attorney has been given delivers the form on behalf of the customer, the power of attorney requires two (2) competent adult witnesses. In this case, we verify the identity of the authorised person in connection with the delivery of the power of attorney.
You can always cancel the authorisation if you wish. Cancellation must be made in writing by delivering the cancellation notice to an Aava clinic. You can cancel the authorisation by using this form template or by sending a free-form cancellation notification.
