Patient data rectification request
You have the right to request the rectification of incorrect data or the completion of incomplete data. You can also request the erasure of unnecessary data from Aava’s patient register. Please note, however, that Aava has a statutory obligation to store patient data in order to safeguard the rights of the customer and Aava. According to the Decree on Patient Records, the period for which patient documents must be stored is 12 years after the patient’s death or, if unknown, 120 years from their birth.
Each request for correction, supplementation and deletion is processed and assessed by a health care professional together with the Chief Physician. The request for correction, supplementation and deletion of patient data must be made in writing by using this form template or a free-form request. The request must be delivered in person to an Aava clinic. Your identity will be verified when submitting a request.
The request for rectification will be implemented without undue delay, but no later than one (1) month after receipt of the request.
Aava also has the right to refuse a request for rectification, supplementation or deletion. In this case, Aava must provide a written certificate of refusal stating the reason for the refusal.
If you are not satisfied with Aava’s decision regarding the correction, supplementation or deletion of your data, you have the right to refer the matter to the Office of the Data Protection Ombudsman, which acts as the supervisory authority, or to use other legal remedies.
