Checking and release of patient records
You can easily check your data in the Oma Aava service or MyKanta. You can also request your patient data in paper format by using this form template or a free-form request. The request must include the following information:
- your name
- identity code
- clinic, and
- the period of time for which data is requested
- details of the data requested (e.g. medical records, laboratory test results, imaging examinations)
- to whom the data will be disclosed (to the customer or to another health care unit, for example)
Confirm the request with your signature.
Checking a minor’s patient data
We take into account the age and level of maturity of minors when disclosing their patient data. If the minor is able to decide on their treatment, they should, in principle, request their own patient data themselves. However, a child or adolescent who is able to make their own decisions can authorise an adult of their choice to take care of matters related to their treatment on their behalf with a power of attorney for acting on behalf of another person.

Release of patient records
We always verify the identity of the person requesting the release in connection with the release of patient data.
If another person collects your patient data on your behalf, they must have a photo ID and a power of attorney with which you authorise the person in question to collect the requested data. The free-form power of attorney for the collection of patient data must include:
- the name, identity code and signature of the person requesting the release, including their signature and name in block letters, and
- the name and identity code of the person collecting the patient data and information about what data the person in question is permitted to collect, as well as their signature and name in block letters
Two (2) competent persons must confirm the power of attorney with their signatures.
If you live far away or are unable to personally collect the patient data, the requested data can be sent as a registered letter to be collected from the post office. In this case, the letter must be picked up personally. The price of this service is EUR 20 and it is charged to you on an invoice attached to the letter.
