Request for the inspection and clarification of log data
Inspection request
If you want to know who has processed your data, you must submit a written log data inspection request. You can submit the request by using this form template or a free-form request. The free-form request must include the following information:
- your name
- identity code
- contact information, and
- the period of time for which data is requested
Clarification request
If, based on the log data you have inspected, you suspect that your data has been processed unlawfully, you can submit a clarification request. The clarification request must be made in writing by using a form template or a free-form request. The free-form request must include the following information:
- your name
- identity code
- time of suspected misconduct
- how the suspicion arose
- what kind of data the request concerns, and
- who you suspect of the misuse of your data
The signed log data request can be mailed to Aava Medical Centre/log data requests, Myllytie 1A, 04410 Järvenpää or sent via encrypted email to [email protected]. We strive to contact the customer within two (2) weeks of receiving the log data request.
In accordance with the law, you have the right to receive your log data from the previous two (2) years. We primarily recommend that you check the log data in a meeting with an Aava employee. This way you will have the opportunity to ask questions about the log data. Alternatively, the log data may be provided to you in writing to the address specified in the request. We will verify your identity from a photo ID before disclosing log data.
Aava has the right to refuse a log data inspection request, for example, if the disclosure of log data poses a risk to the customer’s health or treatment or the rights of someone else. In this case, Aava must provide a written certificate of refusal stating the reason for the refusal.
